• Call the Customer Care Center at 1-888-439-3633 seven days a week: 8 a.m. to 11 p.m. Eastern Time Monday through Saturday, and 11 a.m. to 8 p.m. Sunday.
• View benefits, check eligibility and use other automated services.
• Locate providers through the Customer Care Center, and automated information line.
• Order replacement contact lenses, with valid prescriptions, at competitive prices and have them mailed directly to their homes.

National network provides real savings

With a HumanaVision plan, your employees will have access to the extensive EyeMed network: 40,000 national providers at 20,000 locations, including optometrists, ophthalmologists, opticians and some of the most recognizable names in eye care: LensCrafters®, Pearle Vision®, Sears Optical, Target Optical, and JCPenney Optical.
